Endocrine and metabolic dysfunction in polyendocrine metabolic ovarian syndrome: An overview

Abstract
Polyendocrine Metabolic Ovarian Syndrome (PMOS) is an emerging concept that expands the understanding of conventional Polycystic Ovary Syndrome (PCOS) by emphasizing its multisystem endocrine and metabolic nature. Rather than being confined to ovarian dysfunction alone, PMOS involves complex interactions among the hypothalamic–pituitary–ovarian (HPO) axis, insulin signaling pathways, adrenal glands, thyroid function, pancreatic endocrine activity, adipose tissue, and inflammatory mediators. These interconnected abnormalities result in reproductive dysfunction, metabolic disturbances, cardiovascular complications, and long-term endocrine disorders. Hyperandrogenism, insulin resistance, obesity, chronic low-grade inflammation, dyslipidemia, oxidative stress, and altered adipokine secretion are central features contributing to disease progression. Emerging evidence also highlights the role of gut microbiota, mitochondrial dysfunction, and epigenetic modifications in disease pathogenesis. Understanding PMOS as a systemic endocrine-metabolic disorder facilitates earlier diagnosis, individualized therapeutic interventions, and improved long-term outcomes. This review summarizes the endocrine abnormalities, metabolic dysfunctions, underlying molecular mechanisms, associated complications, and current therapeutic approaches in PMOS.
